Medical Home is a service Southpoint provides for our "Super Kids". These are patients with special medical and developmental needs. We will be choosing a child to spotlight and introduce to you, our Southpoint family. If you have a child that qualifies for Medical Home and would like your child to be spotlighted, please call our Medical Home Coordinators at 801-565-1162 ext. 170
Qualifications for Medical Home include, but are not limited to:
Autism Spectrum Disorders
Diagnosed Behavioral Disorders
Neurological Disorders
Developmental Delay Disorders
Muscular Disorders
